Case Study #2

MR is a 45-year-old male who was inspired by the Biggest Loser show to start exercising.  He went to his doctor who agrees with his decision to start exercise and referred him to your gym.  He is obese (~375+ lbs.) and gets short of breath just walking from the car to the door.  He has some knee pain but thinks it may be due to his weight.  He is very determined to start exercising.  Can you help him with his program?

Age:  45 years old      Height: 5’9″       Weight:  He thinks ~375 lbs.

Received clearance from his MD – Apparently healthy (BP, glucose, lipids all within normal limits)

Family history:  His mother (deceased) was a Type 2 Diabetic, overweight, had high blood pressure and osteoarthritis in both knees; His father had high blood pressure and  passed away from a heart attack at 75 years old.

Orthopedic concerns:  States that he his knees bother him especially when he walks longer than 5 minutes. 

Commitment:  He is willing to exercise 4-5 days/week after work (he works from 8-4:30PM) and his job is very sedentary.  He states he drinks a lot of coffee too.

Question 1: (5 points)

What type of assessment would you perform on MR and why? Explain what test (ONLY ONE) you would use and why is it important.  Consider his weight, orthopedic situation and lifestyle.

Question 2: (2.5 points)

Create a cardiovascular exercise program for MR.  How long (time) would you have him exercise?  How many days/week?  What type of aerobic activity would you recommend?  If you do recommend equipment, be mindful of his weight.  [ADDED INFORMATION:  MR does NOT like to swim, so water exercises are not encouraged.]

Question 3: (2.5 points)

Would you include a strength training program for MR?  If so, what kind/type of exercises would you perform?  Again, be mindful of his weight and orthopedic concerns.  Recommend 4 strength exercises (can be body weight, “toys” and/or machines), repetitions, sets and days/week. (2.5 points)
